New research on informational continuity for patients transitioning into long-term care

Augustine Okoh is a Health Policy doctoral student working under the supervision of Lawrence Grierson. Along with collaborators Henry Siu, Michelle Howard and Keya Shah and others, Augustine is initiating new research that contemplates informational continuity as aging patients move into long-term care. This program of inquiry will map the evidence elucidating the conceptualization of effective LTC transition, highlight the ways in which family physicians promote informational continuity for patients transitioning to long term care, determine the information that long-term care teams feel is most important in supporting new patients, and develop a framework for enhancing informational continuity during care transitions. This work aims ultimately to improve the transition experience for patients and providers and to inspire new collaboration between family physicians, LTC practitioners and care of the elderly researchers. Augustine received DFM graduate student pilot funding to support this project.
